Physical activity health screening is essential to identify individuals susceptible to exercise-related health issues. In this activity, you will learn to apply the 2019 PAR-Q+ questionnaire that could benefit a hypothetical patient, whether they undertake regular exercise or not.

Create a hypothetical patient for this exercise, including information about their health, lifestyle, and exercise habits.

Apply the first page of the 2019 PAR-Q+ questionnaire to your hypothetical patient, noting down their potential responses.

Determine, based on your hypothetical patient's responses, if you would need to proceed with pages 2 and 3 of the questionnaire.

Apply pages 2 and 3 to your hypothetical patient, even if it is not necessary according to their responses. This is for your practice and to familiarize yourself with all aspects of the questionnaire.

Based on page 4 of the questionnaire, brainstorm what recommendations you might provide to your hypothetical patient if necessary.

Evaluate if your hypothetical patient might be interested in implementing physical activity changes, based on your interpretation of their responses. In the next lesson, you will learn to write a physical activity prescription. Although your task will be only to write the prescription, you will have an OPTIONAL assignment to implement the prescription based on your patient's hypothetical interest.

Write a summary of this exercise, including all steps, your hypothetical patient's responses, your decisions based on these responses, and any recommendations you made. Reflect on how well you feel you applied the questionnaire and any areas where you felt unsure.
