In this activity, you will dive into identifying and addressing behavior changes for better health. This exercise is designed to be self-driven, allowing you to analyze your current health behaviors and create a personal action plan to improve. Follow the steps outlined below:

Understanding Your Current Health Behaviors: Use available health records or your own knowledge of your personal lifestyle risk factors for this part.

The wellness-health wheel: Replace the wheel titles with health issues such as exercise, healthy nutrition, stress, sleep, smoking, social life and relationships, career satisfaction, etc. You can download a picture of the “health wheel” online or draw one for your own use. Fill in your degree of satisfaction (1 - low, 10 - high) for each of these health issues. Refer to this resource regarding the health wheel: Balance Wheel

Reflection: Reflect on the findings based on the health wheel. How do they make you feel about your health? Which health achievement provides the most satisfaction? Use Appreciative Inquiry Questions to investigate your successes. Identify what you would like to change; this personal decision predicts higher adherence to the change process.

Create an Action Plan according to S.M.A.R.T. principles: Use materials from the module, for example from the Centre for Excellence in Primary Care and the University of California, San Francisco. Remember to choose one small step at a time. Do not attempt large steps, as they increase the probability of failure. Fill the worksheet provided below accordingly; keep it handy as a roadmap to your health improvement.

Assess ‘Importance’ and ‘Confidence’: Reflect on why your chosen behavior change is important to you, and rate your confidence in implementing the change.
