Thomas's Strengths:
- He is kind to his nephew and respectful and cooperative to his sister.
- He wants to help to make his sister make it on her own in a new area and new job.
- He is reliable and responsible with regard to his commitment to his sister and his nephew.
- He is aware that he is at risk for diabetes due to genetics and his weight gain.
- He is intelligent and able to articulate his problems at school, his living situation and hsi feelings about himself and his experience at school.
- He is young and of an age where he has time to make positive changes in his physician and mental health.
- He made the decision to come for help as requested by his sister.
Concerns for Thomas:
Physical concerns:

He is at risk for developing diabetes and cardiac problems.He is obese and has a history of diabeters in his family. His mother died young of diabetes and high blood pressure.
Mental Health Concerns:
- He is at risk for suicide as he appears depressed about his weight, health, and being bullied at school. He expresses hopelessness and suicide ideation.
- He does not communicate his feelings with his sister
- He spends a lot of time watching Television and does not seem to have any other interests or hobbies of his own
- He does not have friends and says that his little nephew is his only friend
- He has moved and is isolated from community or social connections
- He expresses distrust in a previous health care provider
